The forms listed below need to be completed when applying for retirement benefits:
1. Government of Belize Pension and Gratuity Form Check List: used as a guide when preparing necessary documents when applying for pension and gratuity benefits.
2. Government of Belize Pension and Gratuity Form P&G1: is to be filled by the immediate employer of the officer.
3.Government of Belize Pension and Gratuity Form P&G2: is to be filled by the officer who is reaching retirement.
4. Government of Belize Pension and Gratuity Computation Form (P&G3): is completed by the Treasury Department.
5. Government of Belize Pension and Gratuity Casualty Form (P&G4): is completed by the Treasury Department.
6. Government of Belize Pension and Gratuity Life Certificate Forms
- Government of Belize Pension and Gratuity Form P&G5 General: is to be filled by the retiree
- Government of Belize Pension and Gratuity Form P&G5A: is to be filled by the widow of the deceased retiree
- Government of Belize Pension and Gratuity Form P&G5B: is to be filled by the parent if the child is under 18 years of age; and to be filled by child if 18 years or older.
